We launched this investigation into gastroenterology and related surgery at Mid Yorkshire Hospitals NHS, in response to concerns about the trust's ability to handle allegations of poor clinical performance.

In the course of the investigation, we discovered wider concerns about the trust's management structures.

Our investigation uncovered systemic management failings over a number of years, from the most senior level down. Poor management and a lack of medical leadership resulted in the trust's inability to deal with disputes between doctors and to properly investigate claims of poor performance and huge financial debt.
