Schizophrenia: improvement towards compliance with NICE guidelines
Target
Substantially reduce mortality rates by 2010 (from the 'Our healthier nation' baseline, 1995 -1997) from suicide and undetermined injury by at least 20%.
Rationale
People with a diagnosis of schizophrenia are at increased risk of death due to suicide or undetermined injury. Services that comply with key elements of National Institute for Health and Clinical Excellence (NICE) guidelines for the treatment and management of schizophrenia will assist in reducing this risk and the overall incidence of mortality due to suicide and undetermined injury.
The 2005/2006 community mental health improvement review, carried out jointly by the Healthcare Commission and the Commission for Social Care Inspection (CSCI) included an audit of compliance (in secondary mental health services) with the 2002 NICE guidelines on schizophrenia. This indicator includes elements of a repeat of this audit for 2007/08 in order to assess the progress of trusts from the 2005/2006 national baseline (established through the original audit) and specifically includes measures looking at the provision of cognitive behavioural therapy (CBT), the carrying out of physical health checks and assessments of occupational/vocational status.

Construction
This is a three-part indicator. The three parts will be combined to produce a single overall score. For each part 2007/2008 values will be compared against 2005/2006 data from previous schizophrenia audit to establish a level of performance.
The data collection for the 2007/2008 audit is planned to take place in April/May 2008, using an updated questionnaire provided by the Healthcare Commission. Details of the audit process are available via the following link:
Information on the audit of NICE guidelines for schizophrenia
Trusts should refer to the criterion as referenced below for each standard as set out in the NICE guidance.
Part One - Access to psychological therapies:
Numerator:
Total number of service users sampled who have received at least one session of CBT in the 12 months up to 31st March 2008, or who were offered CBT during the same period or for whom CBT was not applicable (NICE CG1 Criterion 2)
Denominator:
Total number of service users sampled
Indicator (part one):
The indicator is the numerator divided by the denominator, expressed as a percentage
Part Two - Physical health:
Numerator:
Total number of service users sampled who have had at least one physical health check, the content of which is in line with recommended good practice, or who were not applicable in the 12 months up to 31st March 2008 (NICE CG1 Criterion 12 and NICE CG1 1.4.2)
Denominator:
Total number of service users sampled
Indicator (part two):
The indicator is the numerator divided by the denominator, expressed as a percentage
Part Three - Social inclusion:
Numerator:
Total number of service users sampled who received an assessment of their occupational/vocational status in the 12 months up to 31st March 2008 or who were employed or did not want an assessment of their occupational/vocational status (NICE CG1 Criterion 10)
Denominator:
Total number of service users sampled
Indicator (part three):
The indicator is the numerator divided by the denominator, expressed as a percentage.
